ChatGPT gives her three names. A brief description of each. No search results page, no map, no links to compare — just a direct recommendation. If your name wasn't in that answer, she never considered you. Not because she made a judgment call. Because you were never presented as an option.
This is how a growing number of patients now find physicians, and it's why strong Google rankings and a well-maintained website are no longer enough on their own.
The research shift most practices haven't noticed
Healthcare is one of the fastest-growing query categories on AI platforms — patients are using them to find providers, understand diagnoses, research procedures, and navigate insurance questions. A query like "endocrinologist who specializes in Hashimoto's near me" or "OB/GYN accepting Blue Cross in [city]" returns a direct recommendation, two to four names, not a list of results to browse. The AI assistant has already made the decision by the time the patient reads the answer, and they often never look further. If you're not in that answer, you don't get a consolation entry further down the page. You simply don't exist in that patient's decision process.
Why Google rank doesn't translate to AI visibility
This is the insight most physicians and their marketing teams haven't internalized: ranking on Google and being recommended by AI are two separate problems with different solutions. Google evaluates your website's keyword relevance, technical health, and Google Business Profile performance — it's fundamentally a document-ranking system, deciding which pages best match a query. Very few have built all of it out to the depth that lets an AI platform recommend them with real confidence — which is exactly why a physician can rank first on Google for their specialty and still be entirely absent from AI recommendations, while a competitor with fewer Google reviews and a lower search ranking is the one getting named.
What separates the physicians AI actually recommends
The directory layer matters more than most physicians assume — not just having a profile on the platforms AI retrieval leans on most for medical queries, but one that's genuinely complete, with real specialty and procedure detail rather than the bare minimum. A profile that's thin on one major platform makes a practice look less established than it actually is, regardless of how strong its standing looks elsewhere.
Beyond the directory layer, AI assistants are frequently asked about specific conditions and procedures, not just specialties in the abstract — which means a website needs real, substantive content connecting your name to the conditions you actually treat, not a bullet point buried in a general specialties list. Consistency of your identity data across every platform matters for the same entity-resolution reasons it matters for any local business — variations in how a practice name is written create ambiguity that causes AI to undercount or misattribute a practice's presence. Structured data that explicitly tells AI systems what you are, rather than making them infer it from prose, meaningfully increases how confidently a model can describe your specialty and location. And editorial mentions — a contributed column on a hospital's blog, a quote in a regional health story, a feature in a "top specialists" piece — carry more weight than any directory listing, because they're independent confirmation rather than self-reported information.
What we've seen when we compare AI visibility to Google performance
We've run AI visibility scans across a large number of physician practices in mid-size U.S. markets, spanning cardiology, orthopedics, dermatology, OB/GYN, endocrinology, and general internal medicine. The pattern holds across specialties: physicians ranking on page one of Google, with strong review counts and well-maintained websites, are often nearly invisible to AI assistants, while lower-ranked competitors with fewer Google reviews get recommended far more often because they've built deeper condition-specific content and more complete directory profiles.
One well-regarded internist we looked at ranked first on Google for their specialty in their city, with a strong review base and a professionally designed, regularly updated website — and still appeared in only a small fraction of the patient queries we tested across the major AI platforms. A newer competitor practice, with a lower Google ranking and fewer reviews, appeared in the large majority of the same queries. The difference wasn't reputation. It was that the competitor had built out real directory depth, condition-specific pages, and correctly implemented medical schema, and the higher-ranked physician hadn't touched any of it.
The window is still open
Most physicians in most markets haven't started building AI visibility yet. The gap between where you are and where you need to be is real, but it's closeable, and your local competitors have likely done as little about it as you have. The changes involved aren't expensive or technically complex, and they're mostly one-time — a complete directory profile doesn't need to be rebuilt every month, correct medical schema doesn't expire, and a condition-specific page keeps working for years after you publish it.
